EGRIFTA WR quantified human PK and formulation boundary
After single 1.28 mg WR SC administration in healthy subjects: median Tmax 0.15 h; mean Cmax 3831 pg/mL (CV 40%); mean AUC0-inf 1172 pg·h/mL (CV 48%); mean elimination half-life 11 min. Systemic exposure resembled older EGRIFTA 2 mg SC.
Context: DailyMed EGRIFTA WR label, sections 12.3 and 2.1; WR 11.6 mg/vial versus original EGRIFTA 1 mg/vial.
Limitations: The less-than-4% absolute SC bioavailability measurement belongs ONLY to older 1 mg/vial EGRIFTA, not WR. WR and SV cannot be substituted; oral/intranasal/IM PK not established.
Route: SC · WR: Tmax 0.15 h; Cmax 3831 pg/mL; AUC0-inf 1172 pg·h/mL; half-life 11 min. Older 2 mg EGRIFTA formulation: absolute SC bioavailability less than 4%.

Pivotal HIV-lipodystrophy visceral-fat outcomes at 26 weeks
Original EGRIFTA 2 mg SC daily: Study 1 VAT mean change -18% versus +2% placebo; Study 2 -14% versus -2% placebo. Mean weight differences versus placebo were small and not statistically significant in the label tables.
Context: DailyMed EGRIFTA WR label section 14, Table 2 and Table 3; original 1 mg/vial formulation; trials NCT00123253 and NCT00435136.
Limitations: These results reflect HIV-infected adults with lipodystrophy, not generalized obesity or WR-specific efficacy trials; VAT reduction does not imply substantial total weight loss.

Original EGRIFTA adverse reactions and immunogenicity
In original-formulation trials, injection-site reactions occurred in 25% versus 14% placebo, and hypersensitivity reactions in 4% of EGRIFTA recipients. Diabetes defined by HbA1c >=6.5% occurred in 5% versus 1% placebo (HR 3.3, 95% CI 1.4–9.6). Anti-tesamorelin IgG was detected in 50% after 26 weeks, 47% after 52 weeks, and in 85% of the subset with hypersensitivity reactions.
Context: DailyMed EGRIFTA WR label sections 5.4–5.6, 6.1, 12.6; safety extrapolated from original EGRIFTA (1 mg/vial). Antibody positivity is not synonymous with clinical allergy.
Limitations: Percentages are from the older EGRIFTA clinical studies, not direct WR-specific event rates or estimates for research preparations. Antibody testing depends on assay and observation time. The label also warns of elevated IGF-1, edema, glucose intolerance, neoplasm concerns and severe hypersensitivity.

WR indication, non-interchangeability and evidence gaps
EGRIFTA WR is indicated to reduce excess abdominal fat in adults with HIV-associated lipodystrophy; it is not indicated for weight-loss management. WR 11.6 mg/vial and SV 2 mg/vial differ in dose, reconstitution and storage and are not substitutable. Long-term cardiovascular safety is not established.
Context: DailyMed EGRIFTA WR label sections 1, 2.1, 4, 5 and 12.3; dosing context applies to approved product formulations.
Limitations: No verified human PK or efficacy here for oral/intranasal formulations, nontrial peptide vials, alternate cycles, or personalized allergy prevention; do not extrapolate original EGRIFTA safety rates to WR as independently measured.

Fluid retention and IGF-1 monitoring: label-supported findings
Tesamorelin-induced GH secretion can cause edema, arthralgia and carpal tunnel symptoms. Labeling advises IGF-1 and glucose monitoring; consider discontinuation for persistent IGF-1 elevations, for example >3 age-adjusted SDS, particularly without a robust efficacy response.
Context: Approved EGRIFTA WR label sections 4 and 5; safety observations derive from the original-formulation studies. The video discusses these effects at 0:51–1:22, 1:37–3:24 and 6:10–6:43.
Limitations: Active malignancy is contraindicated; prolonged IGF-1 elevation has unknown effects. Glucose intolerance/diabetes and hypersensitivity remain risks. The label does not establish edema resolution at weeks 3–4 or validate a 500 mcg titration scheme. Persistent or progressive symptoms must not be presumed an adjustment period.

Visceral fat benefit can be lost after withdrawal
In a randomized trial of 404 adults with HIV and central fat accumulation, original-formulation tesamorelin was studied for six months, followed by rerandomization to continuation or placebo for six months. VAT reduction reached approximately 18% at 12 months among continuing patients; initial VAT improvements were rapidly lost after switching to placebo.
Context: Falutz et al. 2010 abstract; original EGRIFTA 2 mg SC daily. Video claim at 8:57–9:34 is supported within this population.
Limitations: HIV-associated abdominal fat accumulation; abstract-only trial review. This does not quantify regain for an individual or validate 12–16-week courses and 30-day breaks. Current approved formulations have different labeled doses.

Muscle imaging changes do not establish improved performance
An exploratory secondary analysis found improvements in selected trunk-muscle CT density and area after 26 weeks. It compared 193 tesamorelin responders with 148 placebo participants with available scans; active participants were selected for at least 8% VAT reduction.
Context: Adults with HIV and abdominal adiposity from two prior trials. DOI 10.14283/jfa.2018.45; full text reviewed. Video 7:26–7:41 generalizes to lean muscle preservation or building.
Limitations: Responder-selected secondary analysis rather than an unselected randomized comparison. Muscle imaging does not establish strength, function, athletic hypertrophy, or muscle preservation during tirzepatide/retatrutide treatment.

Deep-sleep GH physiology does not prove bedtime superiority
The major sleep-related GH peak is associated principally with deep, slow-wave sleep, not REM as asserted at 9:41–9:48. A classic physiology study sampled eight young adults over 38 nights; this was not a tesamorelin dosing study. EGRIFTA labeling specifies daily administration without a mandatory bedtime or fasting interval.
Context: General human GH physiology plus selected label administration instructions. Video 9:41–11:02 extrapolates to bedtime, fasting and sleep benefits.
Limitations: No comparative tesamorelin evidence reviewed establishes bedtime superiority, required 2–3-hour meal separation, better sleep, or extra visceral-fat loss from that window. Human physiology is not clinical validation of a timing protocol.
