Cibinetide: early human neuropathy evidence and route-specific PK
A 64-person randomized phase 2b sarcoidosis trial compared 1, 4 and 8 mg/day with placebo for 28 days; 4 mg/day improved corneal nerve fiber area. This is a surrogate endpoint, not proof of general nerve restoration or tendon repair.
Context: Sarcoidosis-associated small nerve fiber loss and neuropathic pain; 2017 trial abstract reviewed. Separate 2015 diabetes paper reports prior healthy-volunteer SC PK.
Limitations: Small, short, indication-specific studies. The 2017 full text was not independently reviewed. No established repeat-cycle interval, general anti-aging benefit, combination efficacy or optimal longer course. Half-life is not absolute bioavailability; a blanket two-minute half-life omits route context.
Route: SC · The 2015 paper reports prior normal-volunteer data after 4 mg SC: peak plasma concentration approximately 3 ng/mL and terminal half-life approximately 20 minutes. Absolute SC bioavailability was not established by this report.
Type 2 diabetes and neuropathic symptoms: 28-day study
49 enrolled, 48 analyzed; serial HbA1c available in 42. Mean HbA1c change at day 28 was -0.16 percentage points with ARA 290 versus -0.01 placebo; at day 56, -0.21 versus +0.21. HbA1c was a primary endpoint. PainDetect improved; corneal nerve findings were restricted to a low-baseline-density subgroup.
Context: Randomized double-blind placebo-controlled trial in adults with type 2 diabetes and painful distal neuropathic symptoms; 28 days of treatment and 28 days of follow-up.
Human study regimen: Studied regimen: 4 mg SC once daily for 28 days, followed by 28 days without study drug. Research exposure, not a recommended regimen.
Limitations: Small, short trial; modest glycemic change, multiple outcomes and subgroup limits. Four serious adverse events occurred in the active arm; two were judged possibly related. Causality is uncertain; the safety committee judged the reported fatal myocardial infarction unrelated. Does not establish long-term safety or a treatment recommendation.
Nine enrolled and eight completed an uncontrolled open-label study. Mean visual acuity, central retinal thickness, retinal sensitivity and tear production did not improve at week 12. Some individual exploratory measures and vision-related questionnaire scores improved. No serious adverse events/reactions or anti-cibinetide antibodies were observed.
Context: 2020 phase 2 pilot in patients with diabetic macular edema; eight completers cannot establish uncommon risks or comparative efficacy.
Human study regimen: Studied regimen: 4 mg SC once daily for 12 weeks. Research exposure, not a recommended regimen.
Limitations: No placebo control; very small sample. Tolerability during 12 weeks does not establish benefit from longer use for glucose control, an optimal duration, or long-term safety.
